Responsibilities

  • Resident Assessments: Conduct thorough assessments of residents' social, emotional needs, and psychosocial health. Develop individualized care plans in collaboration with the healthcare team.
  • Documentation and Record Keeping: Maintain accurate and up-to-date records on all residents regarding social service activities, including progress notes, assessments, and resident interactions. Ensure that all documentation aligns with regulatory requirements and facility policies.
  • Legal Compliance: Ensure compliance with federal, state, and local regulations, including those related to resident rights, confidentiality (HIPAA), and discharge planning.
  • Resident Advocacy: Serve as an advocate for residents, ensuring their personal and property rights are respected. Provide education to residents and families on legal matters such as living wills, advanced directives, and resident rights.
  • Collaboration with Health Teams: Work closely with medical and nursing staff to interpret and address residents' psychosocial needs. Provide key insights on social, psychological, and emotional factors that impact care planning.
  • Complaints and Grievances: Document and resolve resident complaints and grievances, ensuring that proper procedures are followed and outcomes are recorded in compliance with facility policy.
